Family Connects is an evidence-based model that provides whole-person, integrated health support for all families with newborns during a critical life transition. Family Connects nurses are trained to thoroughly assess the health of both newborns and mothers, identifying opportunities for improvement and addressing concerns, including seeking immediate medical care when necessary. These nurses take a holistic approach, considering the needs of the entire family and recommending mental health services or additional medical care for other family members as required. They also follow up to ensure that families’ needs are fully met.
Founded in 2008 through a partnership with the Duke University Center for Child and Family Policy, the Center for Child & Family Health, and the Durham County Health Department, the Family Connects Model has since progressed through various stages, including Consultation, Orientation, Installation, Implementation, Certification, and Expansion.
I recently got to experience the immense benefits of the Family Connects program when I had my daughter. It was such a huge relief to have a nurse come to me during some of the hardest weeks I’ve experienced yet, trying to adjust to my new life as a working mom.
