Parenting is a joyful time of life, but it can also be difficult. Families benefit from support, attention, and encouragement when a newborn arrives. Family Connects Colorado nurses provide in-home support, guidance, and connections to resources at no cost. This new home visitation program will be available to families with newborns beginning October 2022, complementing existing programs and resources already available in Eagle County. A Family Connects Colorado nurse will provide one to three home visits to the family beginning about three weeks after birth, with the aim of enhancing the health and wellbeing of the mom and the newborn by answering questions, providing support and encouragement, and connecting families to helpful resources.

Family Connects New Orleans provides one to three in-home visits, free of charge, to parents of newborns up to 12 weeks old residing in Orleans Parish who give birth at Ochsner Baptist or Touro Hospital. You can expect a visit with a registered nurse around 3 weeks postpartum. The Family Connects model brings together Families, Healthcare Providers, and Community Agencies to give babies their best start possible. Research has shown that when families participate in Family Connects, mothers feel less anxious, parents learn about quality child care options and babies need less emergency care at hospitals. Family Connects North Carolina; Southeastern Region is a community-wide in-home nurse visiting program available to families with newborns in Cumberland, Hoke, or Robeson Counties. Eligible families who sign up will receive up to 12 weeks of support, including an in-home or virtual* visit by a specially trained registered nurse from our nurse partners at Carolina Collaborative Community Care (4C). Spartanburg Medical Center is a Family Connects International Certified Community Partner. The benefits of this certification include additional layers of support for all newborns’ primary caregivers who reside in the city of Spartanburg. This includes foster and adoptive parents. Family Connects links parents to the individual community resources they need regardless of income.

Shortly after the baby’s birth, a registered nurse will visit the home of the newborn, provide health checks for both the infant and the birth mother, assess needs and offer supportive guidance on a wide variety of child and infant health-related topics. The Early Childhood Coalition works to ensure all young children are safe, healthy, and ready for school. We are a group of providers, policymakers, government leaders, experts, parents, and philanthropists supported by Bastrop County Cares, which provides backbone support. Our common vision is that ALL families in Bastrop County are connected to their community and provide a safe, healthy, and school–ready environment for their children. One of the ways the Early Childhood Coalition works to achieve that goal is through a partnership with the Lone Star Circle of Care Family Connects program, funded by The Texas Department of Family & Protective Services and Lone Star Circle of Care. 

The first few weeks after a baby is born can be overwhelming and exhausting for parents, whether the child is their first or they have had more than one child. Lone Star Circle of Care’s (LSCC) Family Connects program supports Bastrop County newborns and their families by creating connections between the family, a specially trained nurse, and community resources. This connection takes place during either in-home visits or telehealth calls. Using an evidence-based approach, the Family Connects program is committed to strengthening the continuum of care and improving health outcomes for mothers and their babies.
